Subject: Additional insured endorsement request — [Project name] Hi [Agent name], Please review the insurance requirements in [Agreement reference] for your insured [Vendor legal name]. Required entities: [Exact legal entity list] Project / premises: [Project and address] Work scope: [Vendor operations] Requested scope: [Ongoing operations, completed operations or other contract-supported scope] Required period: [Work and follow-up period] Please provide the applicable endorsement documents and identify any conditions, exclusions or limitations relevant to this request. If coverage is provided by blanket wording, please identify what written agreement or relationship is needed for it to apply. We also need confirmation of [Other related contract requirement, if applicable]. Please advise if any requested term is unavailable. Return evidence to [Review email or upload link] by [Deadline]. [Reviewer name]
Example in practice
A property owner and management company are different entities. Listing only the building address does not answer whether the required parties fit the endorsement wording.
Certificate holder status is different from additional insured status. Do not treat a correctly completed holder block as evidence that additional insured coverage applies.
