COI Review Checklist
Use the checklist as a document review record. Mark unclear items pending and identify who can resolve them; do not turn unknowns into yes answers.

How to use it
- Keep the contract requirements and latest certificate open side by side.
- Use one review per vendor, project and document version. Retain earlier evidence when renewals arrive.
- Complete the final decision only after required supporting documents and any approved exceptions are recorded.

Example in practice
A COI has current dates and the correct holder, but the insured entity is a similarly named affiliate. The checklist leaves the identity issue pending until the agent explains the relationship.
A complete checklist is a record of your review, not a guarantee that every loss is covered.
