Insurance Broker Document Request Checklist

Ask for the smallest set of documents that resolves the review question. Describe why each item is needed so the broker can return the correct policy period and form.

How to use it

  1. Attach the relevant agreement requirements and identify the insured, project and work dates.
  2. Request specific endorsements or excerpts instead of an indiscriminate full-policy request where a narrower record is sufficient.
  3. Track each requested item and the reviewer responsible for deciding whether it resolves the gap.

Example in practice

A pollution liability question may require the relevant endorsement schedule and wording. Another copy of the one-page GL certificate will not answer that question.

Handle policies and underwriting records as confidential business documents. A broker’s explanation should be retained alongside, rather than substituted for, the applicable policy evidence.
