Vendor Insurance Requirements Template
Use a structured schedule for service vendors with different exposures. Choose requirements with your broker rather than copying the highest limits from an unrelated contract.

How to use it
- Describe the actual work, locations, vehicles, hazardous activities and any subcontracting.
- Have the risk owner specify limits, forms and insured parties; identify any owner or lender requirements that flow down.
- Attach the approved schedule to the agreement and send the same version to the agent reviewing the insurance.

Example in practice
A maintenance vendor who only changes filters has a different work scope from one modifying refrigerant systems. The schedule captures that distinction before limits are selected.
This schedule is a drafting aid. It does not establish statutory limits, amend a policy or replace an approved contract exhibit.
