How to use it
- Use the audit period in the insurer notice and gather all subcontractor payments within that scope.
- Connect each invoice or payment to work dates and the relevant policy or other accepted evidence.
- Record disputed items and the auditor’s requested follow-up without assuming a certificate eliminates a charge.

Example in practice
A subcontractor supplies a current certificate in November for work completed in May. The reviewer needs evidence applicable to May, not just the latest renewal.
Audit treatment depends on insurer rules, state rules and actual work records. Do not fabricate a labor allocation or backdate insurance evidence.
