Describe the document gap precisely and state what evidence would resolve it. This creates a clearer review trail than a generic rejected status.

How to use it
- Quote the requirement from the actual agreement and compare it to the submitted evidence.
- List one issue per line. State missing or unconfirmed where you lack evidence instead of declaring the vendor uninsured.
- Have the vendor route policy or endorsement changes through its agent and insurer.

Example in practice
A certificate shows a waiver indicator but no requested endorsement copy. Ask for that document and its applicable policy rather than demanding that the indicator be changed to yes.
Do not treat a certificate correction as proof that a policy was changed. Check the underlying endorsement when the requirement depends on policy terms.
