Certificate of Insurance Request Email Template
Ask for the evidence needed to assess a vendor before work starts. Give the broker enough information to issue a certificate accurately instead of asking the vendor to change the PDF themselves.

How to use it
- Confirm the contracting entity, project address and insurance exhibit before sending. Use the legal certificate holder name from your contract.
- Attach your actual requirements or paste a link. Request endorsement copies when your agreement requires them.
- Name one contact for corrections and set a date that leaves time for review before mobilization.

Subject: Insurance documents needed before work at [Project name] Hi [Vendor contact], Before your scheduled start on [Start date], please ask your insurance agent to send current evidence of insurance for [Vendor legal name]. Certificate holder: [Certificate holder legal name] [Certificate holder address] Project and scope: [Project name and work scope] Insurance requirements: [Requirements attachment or link] Please include the additional insured and waiver endorsement documents required by that agreement. A certificate checkbox alone may not establish their terms. Send the documents to [Review email or upload link] by [Document deadline]. Please tell us if any requested coverage or wording is unavailable so we can resolve it before work begins. Thank you, [Sender name]
Example in practice
For a tenant improvement, identify both the hiring entity and premises. An agent who receives only a building nickname may issue the holder block to the wrong entity.
Only request wording supported by the policy. If the broker cannot meet a requirement, route the gap for review rather than asking for unsupported certificate language.
