COI Tracking Spreadsheet: Excel and Google Sheets Template
Keep one row for each policy, rather than one expiration date for an entire vendor. The Excel workbook calculates date-review statuses; you still record your insurance-document review and approval separately.
Download the original CSV template
How to use it
- Download the Excel workbook for date formulas, or import the CSV into Google Sheets using File → Import. For the workbook, set the review date and reminder window above the table.
- Replace the clearly labeled example rows. Add GL, auto, umbrella and workers comp as separate rows under the same vendor, using dates from the actual policy evidence.
- Filter the date review column for missing information, future coverage, expired policies and upcoming renewals. Record endorsement evidence and your reviewer decision separately. Copy a prepared blank row when extending the workbook.

Example in practice
A roofing vendor renews GL in January and workers comp in February. Two rows keep the February renewal visible after the January COI arrives. A current date alone does not establish that roofing operations or the owner are covered.
The CSV has no formulas. The Excel date status checks dates only; it does not approve coverage, verify authenticity or replace a review of the contract, policy and endorsements.
