How to Request a Certificate of Insurance From a Vendor
To request a certificate, give the vendor or agent the agreement requirements, accurate certificate holder details, work scope and a submission deadline. Ask for the supporting policy documents needed for your review, rather than only saying “send insurance.”
Prepare the request
Confirm the contracting entity and the parties named in the agreement. A property address identifies a location; it may not identify the legal entity that needs evidence. Review the insurance exhibit with the authorized risk owner before sending it.
Describe the work and dates. A plumber doing routine servicing presents different scope questions from one relocating buried gas lines. If the request includes additional insured or waiver requirements, identify the applicable policies and request the relevant documents. See the broker document checklist.
Send it to the right person
The vendor can forward the request to its agent, or introduce the agent to your review contact. Use a secure, clearly identified upload route and one correction contact. Avoid circulating sensitive policy or claims material in a broad project email chain.
Allow time between the document deadline and mobilization. If the agent cannot meet a requirement, the reviewer needs time to assess alternatives or obtain approval. Do not promise that simply naming your company on a certificate will change the underlying insurance.
A hypothetical request
A facilities manager asks an HVAC vendor for a COI using only a building nickname. The certificate arrives with the wrong holder entity and no evidence addressing a rooftop lift. A better request identifies the contracting company, project address, lifting scope and relevant agreement so the agent can respond accurately.
Review the response
Check the named insured, each policy date and relevant limits against the request. Resolve entity discrepancies and obtain required endorsements. A certificate is a summary, so policy-specific questions can require further evidence. New York OGS explains relevant insurance terminology; use the applicable jurisdiction and actual documents for your decision.
Follow up specifically
Use a deficiency email naming each remaining item, the observed evidence and what would resolve it. Preserve the original document and the response. Once reviewed, record each renewal date and use the request email template for the next vendor rather than recreating the instructions from scratch.
Frequently asked questions
Should I ask the vendor to edit the certificate PDF?
No. Ask the vendor to obtain corrected or additional evidence through the appropriate issuing agent or insurer. A recipient should not alter the evidence.
What if the vendor cannot meet a requirement?
Record the exact gap and send it to the authorized risk or contract reviewer. An exception or amendment requires the approvals in your agreement.
